Is There Someone Under Age 25 Facing a Chronic Health Condition on Your Plan?
For young adults and children In Ohio, there is a program called the Complex Medical Help Program (CMH) managed by the Ohio Department of Health to help diagnose and treat chronic and severe health conditions. It links individuals with special health care needs to a network of quality providers and helps individuals and their families obtain payment for needed services.
The program is available to those who are under 25 years old, are permanent Ohio residents and are under the care of a CMH-approved doctor who have or may have chronic medical conditions such as diabetes, cancer, cystic fibrosis, hearing loss, metabolic disorders, cerebral palsy, and other conditions.
The program helps in three ways:
- Diagnostic program – Children can receive services for up to six months from CMH-approved providers, to rule out or diagnose a special health care need or establish a plan of treatment. This is portion is available regardless of financial need or coverage status.
- Treatment program – Children can receive services for one year for treatment of an eligible condition. Both medical and financial eligibility are required. The program can reimburse for services such as therapies, prescriptions, physician visits, lab tests, and X-rays.
- Care coordination program – This portion is limited to children with specific conditions seen by a team of providers at approved providers.
The state of Ohio receives funding for services from the federal Maternal and Child Health Block Grant, state revenues, county tax funds, and third-party reimbursements and donations.
